Travel Physical Therapist Home Health jobs in Hilo, HI are
available for September start with 13-week assignments. You will
work Monday through Friday, 8-hour days, for 40 hours per week,
covering 25-30 points weekly. Hawaii licensure, home health
experience, and proficiency with Homecare Home Base software are
required.Your responsibilities include evaluating patients in their
homes, developing individualized treatment plans, implementing
therapeutic interventions, educating patients and caregivers, and
documenting visits. You will collaborate with a multidisciplinary
