1/26/2026 Position summary The Registered Nurse in the Transitional
Care Unit provides direct nursing care to adult patients requiring
continued recovery and rehabilitation following acute illness or
surgery. This role focuses on individualized care planning, complex
equipment management, and coordinated discharge planning in
collaboration with the interdisciplinary team. Key responsibilities
Provide direct nursing care to TCU patients in accordance with
hospital standards, policies, and protocols. Assess, plan,
implement, and evaluate individualized patient care plans. Manage
and monitor medical equipment including wound vacs, chest tubes,
NG/G-tubes, tracheostomies, and PICC/PORT lines. Administer
medications and treatments accurately using Pyxis and Meditech/EPIC
EMR systems to support safe medication management and
documentation. Collaborate with physicians, therapists, case
managers, and other team members to ensure continuity of care and
safe transitions. Float to Med-Surg and Surgical Pediatrics units
as required, based on patient census and staffing needs. Maintain
accurate, timely documentation and comply with hospital policies,
infection control, and patient safety standards. Required
qualifications & skills Active Hawaii RN License (must be obtained
prior to assignment start). Minimum 2 years of recent acute care
nursing experience (up to 10 years accepted), preferably in TCU,
Med-Surg, or step-down. Current BLS/BCLS certification (ACLS not
required unless agency-mandated). Proficiency with Meditech or EPIC
EMR systems and standard acute care nursing equipment. Ability to
work variable shifts with no guaranteed hours or specific shift
preference; flexibility with weekends and holidays. Strong clinical
judgment, adaptability, and teamwork skills in a fast-paced
environment. Core competencies Transitional and medical-surgical
nursing expertise across adult patient populations. Patient safety,
infection prevention, and quality-focused care. Effective
communication, patient/family education, and interdisciplinary
collaboration. Time management and adaptability in variable,
census-driven scheduling. Professionalism, accountability, and
adherence to regulatory and hospital standards. Position details
Contract length: 13 weeks, 36 hours/week after orientation.
Setting: Transitional Care Unit with float to Med-Surg and Surgical
Pediatrics as needed. Nurse-to-patient ratios and unit resources
